UN Secretary-General Antonio Guterres has warned that the world is entering a “silent pandemic” zone as antibiotic resistance surges to unprecedented levels.
According to the UN chief, the crisis is being “supersized” by overuse of antibiotics in humans and livestock, with drug-resistant infections now near certain to become the leading cause of death globally within the next 4 years.
“Antibiotic resistance is being supersized before our eyes. The science leaves no room for doubt: the world is in uncharted territory, and those medicines are losing power. Resistance rates are rising, infections are getting deadlier, and humanity is in the danger zone of untreatable disease,” Guterres said in a statement.
The World Health Organization said resistance will peak in impact towards the end of this decade — though its knock-on effects for health systems will continue well into 2035.
“Antimicrobial resistance is firmly established and will intensify into a very severe threat in the coming years, with big impacts on surgeries, childbirth, cancer treatment and associated risks of sepsis, pneumonia and bloodstream infections,” the WHO said.
Antibiotic resistance is a natural process and is not caused by one country alone.
But scientists expect it will be exacerbated by poor regulation, self-medication and weak sanitation, while amplifying health and economic crises worldwide.
The deadliest year on record for resistant infections was 2024, following years of over-prescription.
“This exceptional rise in resistance… has the potential to deliver a massive blow to health systems and economies across the world”, said WHO Director-General Dr. Tedros Adhanom Ghebreyesus.
“We are already seeing disruption and devastation from longer hospital stays and failed treatments, and we expect these impacts to increase as resistance intensifies.”
He put health ministries and critical sectors like agriculture, pharmaceuticals and water on notice, saying there was “no time to lose” in getting ready.
Antibiotic resistance occurs when bacteria, viruses and fungi evolve to survive drugs designed to kill them, bringing worldwide changes in treatment protocols and mortality patterns.
“Fuelled by exceptionally high antibiotic use across human and animal health, resistance is expected to strengthen further during the coming years to a critical intensity,” the WHO said.
The UN agency classifies resistance threats as low, moderate, high, or critical — meaning it is set to reach the highest level of the four.
The WHO said there was an “exceptionally high likelihood of nearly 100 percent” that resistant infections will outpace new drug development through 2030.
Resistance can reshape healthcare and economies thousands of kilometres away from the first outbreak. This includes failure of routine surgeries in India, untreatable pneumonia in Africa, disrupted maternal care in Latin America, and rising costs throughout global health systems.
For 2026-2028, WHO forecasts indicate an increased likelihood of above-normal infection rates “across almost all age groups”, alongside drug shortages showing a “pronounced and classic response to the critical antibiotic resistance crisis,” the UN’s health agency said.
The index, which measures resistance rates in common bacteria like _E.coli_ and _K.pneumoniae_, has strengthened from an average 35% above baseline between 2022 and 2024.
In some hospitals, last-resort antibiotic failure rates were more than 60% above average during 2025 and early 2026.
